  3. Medical Examiner Certificate of Drivers<\/li>\n
  4. Vehicle’s Registration<\/li>\n
  5. Pre-Use Inspection\u00a0<\/li>\n
  6. Driver Time Record or ELD<\/li>\n
  7. DOT Vehicle Inspection<\/li>\n
  8. Driver Safety History (Previous Carriers)<\/li>\n
  9. Driver License History<\/li>\n
  10. Any Carrier-Driver Agreement<\/li>\n
  11. Drug & Alcohol Test<\/li>\n
  12. Safety Performance History – Records Request<\/li>\n<\/ol>\n

    To reserve a badge\/ID<\/strong> we need the below listed driver and assistant information:<\/p>\n

    1. Drivers First and Last Name:<\/li>\n
    2. Date of Birth:<\/li>\n
    3. Last 4 Digits of Social Security Number:<\/li>\n
    4. Email Address:<\/li>\n
    5. Company phone number:<\/li>\n
    6. Site (sorting center) YOUR CITY & STATE:<\/li>\n
    7. Driver photo- provide a clear headshot using the guidelines below (Please be sure to name the photo as the name of the driver)<\/li>\n<\/ol>\n

      DRIVER PHOTO GUIDELINES:<\/strong>
      Badge Photos should:<\/p>\n

      • Include the entire head as well as shoulders in a forward facing shot (no special angles)<\/li>\n
      • Not a picture of driver license.<\/li>\n
      • Have a solid white background<\/li>\n
      • Not include any hats or non-religious headwear, No Sunglasses, and No non-religious face coverings.<\/li>\n
      • Not include any filters or be a selfie<\/li>\n<\/ul>\n
