   12. DOT Physical/M.E. Certificate (REQUIRED IF DRIVING A BOX TRUCK MORE THAN 10,000 LBS OR LARGER VEHICLE)
   13. DOT Certified Medical Examiners (REQUIRED IF DRIVING A BOX TRUCK MORE THAN 10,000 LBS OR LARGER VEHICLE)
   14. USCIS Form I-9
   15. Safety Performance History – Records Request (NOTE: ONLY IF YOU WERE EMPLOYED BY A COMPANY)
   16. Driver Qualification File Checklist (NOTE: GIVES DETAILS OF REQUIREMENTS)

1. Cargo Insurance of $150,000 with GATSCOMP as the certificate holder. (NOTE: SDDC ONBOARDING)
2. INSURANCE CERTIFICATE THAT REFLECTS:  $1,000,000 Automobile per occurrence, $1,000,000 General Liability per occurrence & $2,000,000 aggregate
3. Dedicated Contract Carrier  Agreement (NOTE: SDDC ONBOARDING)
4. 3th Party GPS Tracking: I need the 3th party GPS tracking info…if it is an app, need the an account created or if it is a device that is integrated on the vehicle I need to be able to access it. PER SDDC requirement

7. Driver photo- provide a clear headshot using the guidelines below (Please be sure to name the photo as the name of the driver)

DRIVER PHOTO GUIDELINES (Access to facility like Amazon): Badge Photos should:

• Include the entire head as well as shoulders in a forward facing shot (no special angles)
• Not a picture of driver license.
• Have a solid white background
• Not include any hats or non-religious headwear, No Sunglasses, and No non-religious face coverings.
